Found it, but wanted to investigate just a little further before I send it in (see Matthew's post) - and then the first combat engagement I had killed me.

So no savegame, but then the glitch didn't rear its head in the 3 jumps I did.
I presume that you havent used any mods? Sometimes mods can introduce strange effects.
No mods, just straight out-of-the-box FTL V.1.03.1.

Yeah, especially since I lost a 2nd guy that way as well, and made damn sure this time around I did not miss any in-game event that would cause that.
Does it continue after you reload it?
Apparently it didn't. Like I said above, after continuing the game now, I was able to make 3 jumps with no problem, then got destroyed.
Does it happen in every ship with a teleporter?
I'll see about that. Until yesterday, I've never had this happen with either Kestrel, Torus, or Fed A (which are the only ships I've run with so far) - and I make quite liberal use of teleporters whenever I can.

I don't really expect this bug to show up again, especially if I'm apparently the only one who's ever had it. If it does make another appearance, I'll holler.

Not to worry, the purpose of that run was just to get a certain Fed Cruiser achievement, which I did.

It was a pretty good run up until people started disappearing, but then again, the Auto-Drone that just killed me would've most likely gotten me regardless.
