Bug receiving crew

When one of your crew dies, and the encounter ends before the death animation and removal of the crew member can commence, and upon receiving a new crew member with a full ensemble; the game will place the new crew member into the old one's slot. However, it will still treat your crew as being over capacity, forcing you to dismiss the replacement or one of your standing members.
