Happened to me twice in a row (in one playthrough). The second time there was a quest marker at the wrong end of the fork, so I followed it without paying attention. I was not very pleased with myself when I discovered I had to backtrack through rebel territory. Again.

I propose that all distances are measured in space meters from now on. 
I've done something like that once, in Far Cry 2. The game came with a map of the game world, so I took out a couple of paints and painted dots on the safe houses and enemy camps I found (green for safe houses and red for enemies). It was really handy!
