I've run multiple games off of my PC with no problem - but "Breach.exe" triggers Sophos antivirus as a "PUA" or "Potentially Unwanted Application".
I installed through Steam. Would it be possible to rename the exe with a hotfix to avoid triggering antivirus? itb.exe maybe

You should be able to allow programs like Into the Breach though Sophos: https://community.sophos.com/kb/en-us/122070
You can also allow it at any time through the Sophos dashboard under "Antivirus Protection Exceptions".

That works fine for Sophos Home, but not for Enterprise. My only PC is issued by my employer so I'm SOL on filing my own exceptions.
Other named *.exes work fine, it's just this one that Sophos has a problem with. There are workarounds but in this case that means filing an IT ticket.
I reckon the guy who said the EXE is missing has this issue.
