Well I just got this with fully upgraded engines ofc AND a master helmsman and master engine operator at their stations with no system damage....
Well I don't know if I should play the lotto or something :p

I got this courtesy of a very laser heavy Rebel Cruiser just before the endgame. In a frenetic fight i noticed the pop-up informing me of my 'achievemnet'
The weird thing is i DID NOT have max upgraded engines. I stopped upgrading after the improvements became smaller, but with 2 aces in the cockpit and engine room, my evade chance was 45%
I think the game only needs you to have evasion at 40% or 45% and doesn't care if you have that from engines or engines + crew ability. Wheter that's a bug or a feature doesn't really matter. Very rare achievement to unlock.
